29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent: a) In the Production Area, on or about and at times prior to October 23, 2012, an employee operating the Mount Blower, filling pillows and pet beds was exposed to a dose of 99.93% or an equivalent dBA (decibel "A" scale) of 90.0 dBA. The sampling was performed 471 minutes during one shift on October 23, 2012. The effect of exposure to loud noise is progressive hearing loss. b) In the Production Area, on or about and at times prior to October 23, 2012, an employee operating the Mount Blower, filling pillows and pet beds was exposed to a dose of 101.6% or an equivalent dBA (decibel "A" scale) of 90.1 dBA. The sampling was performed 474 minutes during one shift on October 23, 2012. The effect of exposure to loud noise is progressive hearing loss. c) In the Production Area, on or about and at times prior to October 23, 2012, an employee operating the Mount Blower, filling pillows and pet beds was exposed to a dose of 122.0% or an equivalent dBA (decibel "A" scale) of 91.4 dBA. The sampling was performed 465 minutes during one shift on October 23, 2012. The effect of exposure to loud noise is progressive hearing loss. d) In the Production Area, on or about and at times prior to October 23, 2012, an employee operating the Mount Blower, filling pillows and pet beds was exposed to a dose of 179.2% or an equivalent dBA (decibel "A" scale) of 94.2 dBA. The sampling was performed 468 minutes during one shift on October 23, 2012. The effect of exposure to loud noise is progressive hearing loss. ABATEMENT CERTIFICATION IS REQUIRED
