29 CFR 1904.29(a): A log of all Work-Related Injuries and Illnesses (OSHA Form 300) and/or the Summary of Work-Related Injuries and Illnesses (OSHA Form 300-A), and/or the Injury and Illness Incident Report (OSHA Form 301) or equivalent forms were not kept by the establishment: (a) Location - 85 Commercial Street, Medford, MA: On or about 10/24/12, the employer failed to record a left shoulder injury that occurred on 8/18/12 which resulted in lost work days. Accardi Foods, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 312792013, citation number 02, item number 001 and was affirmed as a final order on 4/7/09, with respect to a workplacce located at 85 Commercial Street, Medford, MA.
