29 CFR 1926.417(b): Equipment or circuits that were de-energized were not rendered inoperative: (a) As observed on or about 10/23/2012, the electrical breaker controlling the high intensity discharge lamps at the Mirabay Tennis Courts were not locked or tagged out. This exposed the employees to electrical shock when they were changing the lamps.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a)As observed on or about 10/23/2012, two employees that were operating an JLG Lift (Model 460 SJ) were observed without any kind of personal fall protective devices. The employees that were changing the high intensity discharge lamps at Mirabay Tennis Courts were exposed to a fall of approximately 20 feet from the ground.
