29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous:    (a) Site: The employees were framing a 2 story residential constructed home using a Jenny Air Compressor (model K5HGA-8P) which was plugged into a 25 feet extension cord without a grounding pin, exposing employees to electrical shock.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    (a) Front of residence: Employees were exposed to fall hazards while framing on a 2 story residential constructed home, 22 feet 6 inches above ground level, were not wearing a personal fall arrest system.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    (a) Site: The employees were performing framing on a 2 story residential constructed home and were exposed to 22 feet 6 inches fall hazard without a fall arrest system and the employer did not ensure that the employees received training, instruction and education in the hazard of falls and the procedure to take to minimization of falls.

29 CFR 1910.1200(e)(1): Employers shall develop, implement, and maintain at each workplace, a written hazard communication program which at least describes how the criteria specified in paragraphs (f), (g), and (h) of this section for labels and other forms of warning, material safety data sheets, and employee information and training will be met, and which also includes the following:    (a) Site: The employer did not ensure that employees recieved hazardous communcation training prior to use hazardous chemicals such as gasoline and oil.
