29 CFR 1926.453(b)(2)(v):     A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:    a) On or about 10/24/12 at the Koike-Aronson Plant located at 635 West Main Street, Arcade, NY; employees were installing framing on the roof for the modification and expansion of the existing plant. Employees were using aerial lifts to access the roof and were not protected from falling while entering, exiting or riding the lift.    NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.501(b)(1):     Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    a) On or about 10/24/12 at the Koike-Aronson Plant located at 635 West Main Street, Arcade, NY; employees were installing framing on the roof for the modification and expansion of the existing plant. Employees were not protected from falling approximately 40 to 50 feet.    NO ABATEMENT CERTIFICATION REQUIRED
