29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was an alternative fall protection measure provided:    (a)Pacifica Construction Inc., at 5695 Summerlyn Ct., Windsor, CO 80550: On October 24, 2012 the employer did not ensure the employees were protected from injury, in that employees engaged in decking operations on a wood frame residential construction without the use of fall protection.  This condition exposed the employees to a fall  hazard of approximately 8 feet.
