Workers were exposed to potential fall hazards in that a proper jobsite inspection by a competent person was not conducted.

29 CFR 1926.453(b)(2)(v):     A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  Note:  As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system.      a) Employees were engaged in commercial construction activities building a new systems-engineered metal structure. Workers who utilized a Genie S-45, articulating, extensible, boom aerial lift were exposed to potential 19.0 foot fall hazards in that the workers were not attached to the boom or mast.

29 CFR 1926.760(a)(1):     Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems.      a) Employees were engaged in commercial construction activities building a new systems- engineered metal structure. Workers were working on the upper level exposed to potential 19.0-21.0 foot fall hazards in that a proper fall protection system was not utilized.

Workers who utilized a Genie S-45 articulating, extensible, boom aeril lift were exposed to potential fall hazards from a height of 19.0 feet in that the workers were not properly trained in fall protection criteria.
