29 CFR 1910.134(f)(2): Employee(s) using tight-fitting facepiece respirators were not fit tested prior to initial use of the respirator and annually thereafter. On or about October 11, 2012, in the production area, where employees were using respirators, such as, but not limited to 3M 8515 filtering face piece. Employees were exposed to a respiratory inhalation hazard.
