29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about October 24, 2012, on the roof, the employer did not ensure that each employee working and walking on a steep roof, with unprotected sides and edges of the roof, were protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems; exposing employees to the hazards of falls from an elevation of approximately 9 - 15 feet from roof ridge to unfinished ground below.

29 CFR 1926.503(a)(1):     The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    On or about October 24, 2012, on the roof, the employer did not ensure that each each employee working and walking on a 6 in 12 pitch roof, with unprotected sides and edges of the roof, were provided training and hazard recognize in order to minimize the fall hazards. This condition posed a fall hazard to employees of approximately 9 - 15 feet from roof to unfinished ground below due to a lack of traininig.
