29 CFR 1910.305(g)(1)(iv): The employer allowed the use of flexible cords sets to substitute for permanent wiring: (a) U.S. Postal Service @ 556 Pershing St., Craig, CO 81625: On and before 10/23/12, the employer allowed the use of flexible cord sets to serve as permanent wiring, in that the task lighting, located inside the mail sorting cubicles, was powered by extension cords rather than via permanently fixed wiring.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) U.S. Postal Service @ 556 Pershing St., Craig, CO 81625: On and before 10/23/12, the employer did not ensure that a flexible cord plug, which was used to power task lighting on the south employee mail sorting cubicles, was installed so that the strain relief was utilized. This condition exposed employees to the inner conductors of the flexible cord.
