29 CFR 1910.22(a)(1):  Place(s) of employment, passageways, storerooms, and service rooms were not kept clean, orderly and in a sanitary condition:  On October 24, 2012,  and at times prior thereto, at the Parthenon Hospitality, Ltd dba Ambassador Hotel located at 3100 I-40 West, Amarillo, Texas 79102, the employer failed to ensure that the work place was kept in an orderly manner.  Thus, the employees were exposed to slips/trips and fall hazards.

29 CFR 1910.22(a)(2):  The floor of every workroom was not maintained in a clean and, so far as possible, a dry condition. Drainage areas were not maintained, or other dry standing places should be provided where practicable.  On October 24, 2012, and at times prior thereto, at the Parthenon Hospitality, Ltd dba Ambassador Hotel located at 3100 I-40 West, Amarillo, Texas 79102, the employer did not ensure the Boiler Room work area was maintained in a dry and sanitary condition.  Employees were exposed to slip and fall hazards.

Pool Room junction box,      b.  First Floor junction box,      c.  Third Floor junction box,      d.  Seventh Floor junction box,      e.  Nineth Floor junction box.

29 CFR 1910.305(b)(3)(ii):  Employer did not ensure that all pull and junction boxes were closed by suitable covers securely fastened in place:  On October 24, 2012, and at times prior thereto, at the Parthenon Hospitality Ltd. dba Ambassador Hotel, located at 3100 I-40 West, Amarillo, Texas 79102, the employer did not ensure that the covers to the junction boxes were closed with a cover:      a.  Pool Room cover was missing on junction box,      b.  First Floor electrical room door to junction box was open,      c.  Third Floor electrical room door to junction box was open,      d.  Seventh Floor electrical room door to junction box was open,      e.  Nineth Floor electrical room door to junction box was open.

29 CFR 1910.305(j)(2)(vi):  Receptacle installed in a wet location where the product intended to be plugged into it was not attended while in use (for example, sprinkler system controllers, landscape lighting, and holiday lights) shall have an enclosure that is weatherproof with the attachment plug cap inserted or removed:  On October 24, 2012, and at times prior thereto at the Parthenon Hospitality Ltd. dba Ambassador Hotel, located at 3100 I-40 West, Amarillo, Texas 79102, the employer did not ensure the outlets on the Third Floor roof where flexible cords were connected were enclosed, protected from weather.  Employees were exposed to electrical shock.
