29 CFR 1926.651(k)(1): Daily inspections were not made by a competent person: (a) On or about October 18, 2012, an employee was exposed to cave-in hazards while working next to an unprotected vertical and undercut excavated wall that was 12 feet, 4 inches tall, on top of which earth moving equipment was operating. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").
