29 CFR 1910.22(a)(2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: GLAZE LINE AREA: On or about August 24, 2012, a water hose had been fastened above, and was continuously spraying down onto, the glaze line to cool the tile product as it moved along the line. Some of the water accumulated on the floor next to the glaze line drain, exposing employees to a slip and fall hazard.

29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: On or about August 24, 2013, the employer did not ensure that stacked pallets containing tile were not stacked so high (5-6 tiers) that it created a hazard. This condition caused the pallets to topple over.
