On or about 10/24/12, at site 133-38 Sanford Ave. Flushing, NY    Employees demolishing side walk concrete with Jack hammer were in danger of potential eye injury. Protective safety glasses were not worn.    ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
