29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or 1926.652(c): a) Employees were not adequately protected from the hazard of cave-ins while working in a trench measuring approximately 6.8 feet in depth. GMS Plumbing, Inc., was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1) which was contained in OSHA violation number 109248, citation 1, item 2, issued on December 8, 2011 and became final order on or about January 5, 2012 with respect to a workplace located at 1519 W. Oakton Str., Arlington Heights, IL 60004. No abatement certification or documentation is required for this item.
