29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities six feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. a/ At the job site, employees were installing new felt paper and shingles upon a residential roof without conventional fall protection in place. The roof's slope was measured at approximately 45 degrees from the horizontal or 12:12. The ground to eave height ranged from nine to 15 feet. Inadequate slide guards in the form of 2 X 4 lumber were used in places.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a/ At the job site, two, portable, extension ladders were used to gain access to and egress from a 12:12 sloped residential roof. The side rails belonging to the ladder positioned adjacent to the front door were approximately one foot above the landing. The side rails belonging to the second ladder positioned along the north side of the home were approximately two feet above the eave.
