29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system: JOB SITE: 2901 Magnolia Avenue, St. Louis, MO 63118, at this work site, workers were observed on a tubular welded scaffold, approximately 20 feet 9 inches above ground level, doing tuckpoint work and were not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladders length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: JOB SITE: 2901 Magnolia Avenue, St. Louis, MO 63118, at this work, a worker was observed using a Werner Fiberglass extension ladder to gain access to the tubular welded scaffold and the ladder side rails did not extend at least 3 feet above the upper landing surface being accessed. The worker was exposed to a fall hazard.
