Employees exited the edger work platform by walking down the trash conveyor. This condition exposed employees to the hazard of not being able to immediately exit an area in the event of a fire. b. On or about October 25, 2012, the exit route for the work platform at Big Jake and area surrounded by Big Jake, double edge saw line and trash conveyor was obstructed by conveyors. This condition exposed employees to the hazard of not being able to immediately exit an area in the event of a fire.

On or about October 25, 2012, the operator control box mounted on the west side of the belt conveyor at the scrag line had an unused opening in the front and side of the box. This condition allowed wood dust to enter the box exposing employees to hazards associated with fire. e. On or about October 25, 2012, the breaker box located near the scrag line that provided power to the double edge trim saw and conveyors had an unused breaker opening inside the box. This condition exposed employees to the hazard of contacting energized electrical parts.
