29 CFR 1910.110(e)(4)(iii): Permanent and removable fuel containers were not securely mounted to prevent jarring loose, slipping, or rotating:    (a) Smaller Manufacturing Building - A removable fuel container (liquefied petroleum gas) was not securely mounted to prevent jarring loose, slipping, or rotating while on the Clark forklift  - S/N - C40B-144-930.         In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

(a) Manufacturing Area - Modifications and additions which affected capacity and safe operation were performed by the customer or user without manufacturers written approval, and the capacity, operation and maintenance instruction plates, tags or decals were not changed accordingly for the blue colored Hyster sit down liquefied petroleum gas forklift which had no name plate. (a) Manufacturing shop area (vertical turning lathe) - One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area of the Morando Vertical Turning Lathe from hazards such as those created by flying chips and the rotating chuck.    In accordance with 29CFR 1903.19(c), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. 29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place:      (a) Manufacturing Area - The employer failed to ensure that all nameplates on the powered industrial vehicles were maintained to provide the operator with information on the operating capacities of the forklifts such as, but not limited to the blue Hyster sit down forklift.     Dill Brothers, Inc., was previously cited for violation of this occupational safety and health standard or its equivilant 29 CFR 1910.178(a)(6), which was contained in OSHA inspection number 312567845, citation number 1, item number 2, issued on October 30th, 2008 and became final order on Novemver 10th, 2008.
