29 CFR 1910.333(b)(2)(iii)(A): A lock and a tag were not placed on each disconnecting means used to deenergize circuits and equipment on which work was to be performed: a. On or about 10/23/2012, Power Distribution Building 10, A lock was not placed on a 4160V circuit breaker by the qualified electrician who racked out the circuit breaker for an operations employee (unqualified person) who applied a personal hazardous energy control lock and tag at the circuit breaker for maintenance on a 480V pump which exposes operation employees to electrical arc flash hazards.
