29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by paragraph (l) and shall include the name of the operator, the date of training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation: An employee was operating a forklift and the employer did not provide evidence to show that the required powered industrial training and evaluation had been conducted for the operator. There was no written documentation that included the operators name, date of the training, date of the evaluation and the name of the person conducting the training and/or evaluation.
