29 CFR 1926.501(b)(4)(i):     Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.      a- 8 Empire Blvd, Moonachie, NJ: An employee engaged in exhaust fan removal sustained serious injuries when he fell through an unprotected hole on the roof. Condition noted on or about 10/09/2012.

29 CFR 1926.501(b)(10):     Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.      a. 8 Empire Blvd, Moonachie, NJ:  Employees were exposed to a fall hazard of approximately 20 feet while engaged in the removal of an exhaust fan and roofing work without a fall protection system, on or about 10/09/2012.

Conte Roofing Company, Inc was previously cited for serious violations of this Occupational Safety and Health standard or its equivalent standard, 29 CFR 1926.1053(b)(1): which was contained in OSHA Inspection Number 315040873, Citation # 1, Item # 3, issued on 2/08/2011 to a workplace located at 555 Sunrise Hwy, West Islip NY 11803 which became a final order on 3/07/2011.
