29 CFR 1926.501(b)(13):     Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about 25 October, and at times prior thereto, three workers were performing framing activities on a home under construction at the 1st level top plate and above without the use of fall protection, exposing the workers to fall hazards of ~11 feet and ~13.75 feet to the concrete foundation below.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure fall protection is provided when working on the top plate or above that involves heights above 6 feet.

29 CFR 1926.1053(b)(13):     The top or top step of a stepladder was used as a step:    On or about 25 October, and at times prior thereto, three workers were performing framing activities on a home under construction at the 1st floor level (top plate) in which they used the top step of the two 6 foot tall step ladders to access the level, exposing the workers to a fall hazard of approximately 6 to 11 feet in height.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure larger sized step ladders are used on the job site to allow access to higher levels without using the top step.

29 CFR 1926.102(a)(1):     Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents.    On or about 25 October, and at times prior thereto, three workers were performing framing activities on a home under construction using pneumatic nail guns without the use of eye protection, exposing the workers to eye hazards from flying projectiles or debris.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure eye protection is provided and used when utilizing pneumatic nail guns.    A.L. Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA Inspection# 316094978, Citation# 01, Item# 001 and was affirmed as a final order on 1 May 2012, with respect to a workplace located at 1100 Gross Road, Mesquite, TX 75149.

29 CFR 1926.451(b)(2):     Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and walkway was not at least 18 inches (46 cm) wide.    On or about 25 October, and at times prior thereto, a worker was performing framing activities on a home under construction using a makeshift scaffold platform and walkway that had planks of 2x6 pieces of lumber that were not at least 18 inches wide, exposing the worker to a fall hazards of approximately 13 feet and 9 inches to the concrete foundation below.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure scaffold platforms and walkways are at least 18 inches wide.    A.L. Construction was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.451(b)(2), which was contained in OSHA Inspection# 316094978, Citation# 01, Item# 003 and was affirmed as a final order on 1 May 2012, with respect to a workplace located at 1100 Gross Road, Mesquite, TX 75149.
