29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.        a)  Front porch roof, 5585 Black Street, Pittsburgh, Pennsylvania, 15206:  Observed on or about October 24, 2012, an employee working from a scaffold while engaged in flush pointing on the second floor wall of the building was not protected from falling to the lower level.  The employee was exposed to a fall hazard of approximately 13 feet to the ground below.    b)  East side of building, 5585 Black Street, Pittsburgh, Pennsylvania, 15206:  Observed on or about October 24, 2012, an employee working from a scaffold while engaged in flush pointing on the peak of the east wall of the building was not protected from falling to the lower level.  The employee was exposed to a fall hazard of approximately 20 feet to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:       a)  Residential dwelling, 5585 Black Street, Pittsburgh, Pennsylvania, 15206:  Observed on or about October 24, 2012, employees working from scaffolds while engaged in flush pointing on the exterior walls of the structure were not provided with fall protection training by the employer.  The employees were exposed fall hazards of up to 20 feet while working on the scaffolds.

An employee enaged in flush pointing while standing on the aluminum plank was exposed to a fall hazard of approximately 13 feet to the ground below.      b)  Front porch roof, 5585 Black Street, Pittsburgh, Pennsylvania, 15206:  Observed on or about October 24, 2012, a aluminum plank which was being supported by the top rung of a extension ladder.  An employee egaged in flush pointing while standing on the aluminum plank was exposed to a fall hazard of approximately 13 feet to the ground below.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level:          a)  Front porch roof, 5585 Black Street, Pittsburgh, Pennsylvania, 15206:  Observed on or about October 24, 2012, one end of an aluminum plank was being supported by a step ladder.  Two legs of the step ladder were resting on lose bricks.  An employee who was flush pointing while standing on the aluminum plank was exposed to a fall hazard of approximately 13 feet to the ground below.
