29 CFR 1910.147(c)(4)(i):     Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    At Pacific City Graphics, Inc. : The employer did not develop procedures for controling energy during maintenance or servicing activities of the equipment located in the faciltiy. The company has a written program describing its purpose and when it is required, however no specific procedures were developed for its implementation.

29 CFR 1910.305(g)(2)(iii):     Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:    In the Production Area on the South Wall next to the Braum USA paper folding machine: A flexible cord used to power the Braum USA paper folding machine was plugged into a 220v wall outlet. The cord was pinned behind a metal storage shelf and was not provided adequate strain relief in that the cord was bending to a almost 90 degrees.
