First Floor/Metal Stairway leading to the Patterns Storage Room - On or about November 6, 2012, employees were required to retrieve metal molds using a staircase with handrails that measured (21) twenty one inches in height. Employees were thereby exposed to fall hazards.

Molding Operations Area - On or about October 29, 2012, the employer did not conduct daily inspections of the overhead crane systems to move foundry equipment ( Gantry/Budgit - 1 Ton & Jib/Arrow - 1 Ton). The operating mechanism for the (Budgit) crane's hook had its safety latch missing. Employees were thereby exposed to struck by hazards.

Foundry Operations Areas - On or about October 29, 2012, electrical extension cords were being use as temporary wiring. Employees were thereby potentially exposed to electrical hazards. The areas where these extension cords observed were the following; 1. In the molding area an extension cord was used as temporary wiring to power a rail trolley.

Employees were exposed to the health hazards of working with inorganic arsenic. The following area was found with accumulations of arsenic; 1. First Floor/ Grinding Station - floor sweepings were found to contained 0.0042 % of inorganic arsenic.

The top surface area (center location) of the lunch table contained 0.0956 micrograms of chromium (VI). 2. The refrigerator (Hot Point) door's lower handle contained 0.1304 micrograms of chromium (VI).
