29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2). On or about October 18, 2012, the employer did not conduct a workplace hazard assessment where employees are both conducting and providing support for tree trimming operations, exposing the employees to the hazards associated with trimming trees and removing debris from the site.
