29 CFR 1926.150(a)(4): All firefighting equipment shall be periodically inspected and maintained in operating condition. Defective equipment shall be immediately replaced. a. On the first floor of the new Toronto Middle School where two employees were welding cross bracing, the 3A 40 BC fire extinguisher had been discharged and not inspected before use thereby exposing the employees to a fire hazard.
