29 CFR 1926.502(a)(2): The employer did not provide and install all fall protection systems required by this subpart for employee(s) and did not comply with all other pertinent requirements of this subpart before employee(s) began the work that necessitates the fall protection. (a) Front of Hotel, 3rd Floor Window - On or about June 14, 2012 employees were exposed to fall hazards and they were not protected by their employer providing and installing a safety harness lanyard anchor point for an employee in a hoist area. Employee was exposed to a fall of 22.8 feet.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) South End of Hotel, 3rd Floor - On or about June 14, 2012 an employee was exposed to a 22.8 foot fall. Employee was not protected by training on procedures to recognize, minimize and eliminate exposure to fall hazards.
