29 CFR 1926.502(d)(15)(i): Anchorages used for attachment of personal fall arrest equipment were not designed, installed, and used as part of a complete personal fall arrest system which maintains a safety factor of at least two: a) Bridges: On or about October 25, 2012, the employer did not ensure the job made anchorage device system used at the edge of the bridge under construction was adequate for multiple employees at one time which exposed them to falls up to 10-feet.
