29 CFR 1910.156(c)(1): The employer did not provide training and education commensurate with those duties and functions that fire brigade members were expected to perform, prior to performing fire brigade emergency activities: a) As of 10/1/2012 and 11/6/2012, the employer had not provided fire brigade training for employees who were called to extinguish a combustible dust fire, which generated heavy smoke within an enclosed ventilation duct. The employees had not been trained in the hazards of a combustible dust fire, the risk of accelerating or spreading the fire by trying to open the ventilation pipe at the location of smoldering combustible dust, or in the use of personal protective equipment (PPE) for smoke and fire exposure. Abatement Certification and Documentation is required.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening where they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910: a) On or before 11/6/2012, electrical breaker boxes and shutoff boxes on the west wall of the cutting room were blocked from access by pallet storage. There was not a three foot or more clearance around the boxes, nor a means to reach them. Abatement Certification and Documentation is required.
