29 CFR 1910.178(l)(6): Certification. The employer shall certify that each operator has been trained and evaluated as required by this paragraph (l). The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. a) On or about October 23, 2012 and at times prior to, the employer did not certify that employee who operated LULL 644B-42 equipment to move 2 by 4 supplies, had been trained and evaluated, exposing employee to struck-by and crush hazards. ABATEMENT CERTIFICATION AND DOCUMENTATION ARE REQUIRED.
