29 CFR 1926.453(b)(2)(v):  A body belt or safety harness was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    a) Interstate 88 over Main Street, south side of westbound lane, on or about 10-29-12:  Two employees were working in an articulating extensible boom aerial lift without their safety harnesses connected, exposing the employees to a fall hazard of 44 feet.
