29 CFR 1910.23(d)(1): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails. On or about October 11, 2012 and at times prior thereto, the employer did not ensure that the stairway to the rig platform was equipped with stardard stair railings on both open sides. Employees were exposed to falls of more than 4 feet to the ground below.
