29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a) Quality Building Specialists, LLC @ 2000 Little Raven St., Denver, CO 80202: On and before October 9th, the exposing employer did not ensure an employee wore a protective helmet when exposed to overhead hazards such as, but not limited to, falling tools and materials. An employee involved in hand digging a trench excavation to the depth of approximately 11 feet was not provided a hard hat for protection from falling material into the excavation. The condition exposed the employee to struck-by hazards from tools and materials that may fall into the excavation.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c): (a) Quality Building Specialists, LLC @ 2000 Little Raven St., Denver, CO 80202: On and before October 9th, 2012, the exposing employer did not ensure that each employee working within the trench excavation on the north side of the Flour Mill Lofts was protected from cave-in, in that, the excavation was measured depth of 11' and approximately 5'5" in width and lacked any protective system such as sloping, benching, or a shield system. Employees were hand digging the trench in an attempt to locate a water leak. This condition exposed subcontractor employees to crushing hazards by excavation wall cave-in.
