On or about October 30, 2012, on the west side of the chimney, the 4th level of the scaffold did not have guardrails on the end of the platform or on the open sides of the material platform, exposing employees to an 18 feet fall hazard.     b. On or about October 30, 2012, on the south side of the chimney, the 4th level of the scaffold had a walkway platform without a guardrail, exposing employees to an 18 feet fall hazard.
