29 CFR 1926.21(b)(2): The employer did not instruct employees in the recognition and avoidance of unsafe conditions, regulations applicable to their work environment to control or eliminate any hazards or other exposure to illness or injury. On or about October 29, 2012, and at times prior thereto, employee was working inside of an elevated skid-steer bucket, performing masonry work on a rock-wall at a residential location with address: 12055 Copper Valley, El Paso TX79934. Employee was potentially exposed to the hazards created when operating or working around, on or above a skid-steer loader, such as stuck by, fall from heights, etc.
