29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder is used to gain access. This violation occurred on October 30, 2012 at a work place located at 519 S. Cockrell Hill Rd., Duncanville, Texas 75116; where at least one employee was observed using an extension ladder that did not extend at least 3-feet above the landing of the structure.
