29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a. On or about October 30, 2012, on a low pitch roof, in the front of the structure, an employee performing framing operations was not protected from a 12 feet 4-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system, or any alternate fall protection.    b. On or about October 30, 2012, on a low pitch roof, on the side of the structure, an employee performing framing operations was not protected from a 12 feet 8-inch fall hazard by use of a guardrail system, safety net system, personal fall arrest system, or any alternate fall protection.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stairrail system along each unprotected side or edge:    a. On or about October 30, 2012, the stairway system used to access the 2nd level did not have a stairrail system on an unprotected side, exposing employees to a fall hazard.
