29 CFR 1926.403(i)(2):   Live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by cabinets or other forms of enclosures:  On or about 10/30/12 at Macon, GA: Employees were exposed to live electrical parts while performing demolition work at the following locations:  a.  Employees were exposed to electrical hazards when using an electrical switch to turn on temporary wiring at the demolition site.  The employer failed to install a cover on an electrical switch to protect employees from accidental contact to live electrical parts (120 Volts AC).  b. Employees were exposed to live electrical parts (120 Volts AC). The employee failed to properly protect electrical wiring located on the first temporary lighting fixture at the rear entrance to the first floor. The fixture was at a height of 5 foot above the floor.

The employer did not provide adequate strain relief to prevent pull from being directly transmitted to joints or terminal screws:  a. Main Circuit Breaker Box:  The employer failed to provide adequate strain relief to a romex wire that entered an opening located at bottom of the main circuit breaker box.  b. Electrical Box:  The employer failed to provide adequate strain relief on a romex wire which originated at the main circuit breaker box and entered into the left side of the electrical box.  The electrical box is located below and to the right of the circuit breaker box.   c. Light Switch: The employer failed to provide adequate strain relief on electrical wiring that entered at the bottom of a light switch's electrical box.  The light switch applies power to the building's temporary overhead lighting system.  d. Receptacle: The employer failed to provide adequate strain relief on electrical wiring that entered at the top of a receptacle's electrical box.  The receptacle box is located directly below the light switch for overhead lighting system.
