Red Zip Lift overhead floor-mounted jib crane with 2,000 lbs. P&H hoist used for Niigata HN50A CNC. On or about October 31, 2012, load rating was painted over and not visible from the ground for the crane operator to determine if safe load handling could be performed.    (b) Lake Street Building; ATS module assembly area. Orange Abell-Howe overhead floor-mounted jib crane with 4,000 lbs. P&H hoist. On or about October 31, 2012, load rating was painted over and not visible from the ground for the crane operator to determine if safe load handling could be performed.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)

On or about October 31, 2012 employees were exposed to struck-by and crushed or pinned-under hazards where severely damaged slings were used for heavy-lifting of the steel ATS modules' parts. 29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    (a) 2nd Street/North building; CNC Room. Niigata HN50A Machining Center. On or about October 31, 2012 employees were exposed to struck-by hazards from flying chips and any other parts that could strike-out from the point of operation, where a side door or any other form of partition was missing on the back side of the CNC machine.    (b) 2nd Street/North building; CNC Room. Cincinnati Arrow EMC1000 ERM CNC Vertical Machining Center. On or about October 31, 2012 employees were exposed to struck-by hazards, caught-on hazards, and lacerations resulting from the movement of the internal parts during cleaning on the inside of the CNC where a side panel was removed.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c)

(a) 2nd Street/North building; CNC room. Cincinnati Arrow EMC1000 ERM CNC Vertical Machining Center.   (b) 2nd Street/North building; CNC room. Cincinnati Arrow 500 CNC Vertical Machining Center.     The Jakes Machining and Rebuilding Service, Inc. dba Jake's Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 311249437, citation number 01, item number 001a and was affirmed as a final order on June 25, 2008, with respect to a workplace located at 131 2nd Street, Aurora, IL 60506.    Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

On or about October 31, 2012, access to 3 unmarked 480V electrical circuit breakers was blocked by unused machinery and trashed cardboard boxes.
