On or about October 30, 2012, inside the commercial building, employees utilized an extension cord with a missing ground pin to energize a DeWalt skill saw. The employees were exposed to electrical hazards.

On or about October 30, 2012, inside the commercial building, employees utilized an extension cord without a strain relief device to energize a Makita skill saw. The employees were exposed to electrical hazards.
