29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On or about October 30, 2012, inside the commercial building, employees utilized an extension cord with a missing ground pin to energize a DeWalt skill saw. The employees were exposed to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On or about October 30, 2012, inside the commercial building, employees utilized an extension cord without a strain relief device to energize a Makita skill saw. The employees were exposed to electrical hazards.
