29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: At this establishment, a wooden pallet was used as a temporary, elevated work platform for an employee to work from at a height greater than 20 feet. An employee was raised by the forks of a Caterpillar TH63 excavator to perform painting operations. The use of the platform as a scaffold was not designed, constructed, and loaded in accordance with a qualified person design.
