29 CFR 1926.1053(b)(8):  Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder:    On the guardhouse northern roof edge of Golden Lakes Village located at 1700 Golden Lakes Boulevard in West Palm Beach, Florida:  Employees gaining access to and from a low slope roof approximately 11 ft. 6 inches above a lower level using a portable ladder set up in a location where it was exposed to foot traffic did not have that ladder secured to prevent accidental displacement, on or about October 30, 2012.

29 CFR 1926.1053(b)(22):  Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:    On the guardhouse northern roof edge of Golden Lakes Village located at 1700 Golden Lakes Boulevard in West Palm Beach, Florida:  An employee climbing a Werner aluminum extension ladder to gain access to a low slope roof approximately 11 ft. 6 inches above a lower level was carrying a bucket of tar that could cause him to lose balance and fall, on or about October 30, 2012.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On the guardhouse northern roof edge of Golden Lakes Village located at 1700 Golden Lakes Boulevard in West Palm Beach, Florida:  Employees performing roof work on a low slope roof with unprotected edges approximately 11 ft. 6 inches above a lower level were not protected from falling to that lower level, on or about October 30, 2012.    ROOFING CONCEPTS UNLIMITED/FLORIDA, INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVILANT STANDARD 29 CFR 1926.501(b)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315352872, CITATION NUMBER 2, ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 09/22/2011, WITH RESPECT TO A WORKPLACE LOCATED AT 9045 JOG ROAD IN BOYNTON BEACH, FLORIDA.
