29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    On the east exterior elevation of a McDonalds under renovation located at 3546 NW Federal Highway in Jensen Beach, Florida:   An employee using a Hitachi pneumatic gun loaded with 3 inch nails performing framing work installing Bucking for the store sign anchorage was not provided with eye protection equipment while exposed to a potential struck by eye injury, on or about October 31, 2012.

29 CFR 1926.453(b)(2)(v):  A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:     On the east exterior elevation of a McDonalds under renovation located at 3546 NW Federal Highway in Jensen Beach, Florida:   An employee performing framing work installing Bucking for the store sign anchorage while in the basket of an aerial lift approximately 10 ft. 6 inches above a lower level was not wearing a safety harness with a lanyard attached to the boom or basket of that lift, on or about October 31, 2012.

29 CFR 1926.454(a):  The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards:    On site of a McDonalds under renovation located at 3546 NW Federal Highway in Jensen Beach, Florida:   The employer did not provide training by a competent person to an employee performing framing work from the basket of an aerial lift, on or about October 31, 2012.    PLEASE NOTE:  CERTIFICATION OF ABATEMENT IS REQUIRED FOR THIS VIOLATION.
