29 CFR 1910.304(b)(3)(ii)(A): All 125-volt, single-phase, 15-, 20-, and 30-ampere receptacle outlets that are not part of the permanent wiring of the building or structure and that are in use by personnel shall have ground-fault circuit-interrupter protection for personnel: As observed on or about 11-28-2012, a Northrock Vibrator was connected to a Lincoln Welder SAE 400 welder without employing the protection of a ground fault circuit-interrupter.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws As observed on or about 11/28/2012, an extension cord used to power the Northrock Vibrator was found to have a defective strain relief clamp.
