1926.760(a)(1): Each employee engaged in a steel erection activity that was on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems:    (a) Construction Site: On or about October 31, 2012, and at times prior thereto, employees were exposed to a fall of approximately twenty four feet to the ground below while installing insulation on a metal roof.

1926.761(b): The employer did not train each employee exposed to a fall hazard in accordance with the requirements of this section. The employer shall institute a training program and ensure employee participation in the program:      (a) Construction Site: On or about October 31, 2012, and at times prior thereto, employees were exposed to a fall of approximately twenty four feet to the ground below due to lack of adequate training in the recognition of fall hazards related to steel erection, and the means to control these hazards.
