29 CFR 1926.752(b): A steel erection contractor erected steel without receiving written notification that the concrete in the footings, piers and walls and the mortar in the masonry piers and walls had attained, on the basis of an appropriate ASTM standard test method of field cured samples, either 75 percent of the intended minimum compressive design strength or sufficient strength to support the loads imposed during steel erection activities: (a) High Ball Erectors @ 10081Wadsworth Parkway, Westminster, CO. On or before October 11, 2012, the steel erection contractor did not ensure that the general contractor provided them with the following written notifications prior to commencement of structural steel erection activities on the job site. (1)The concrete in the footings, piers and walls and the mortar in the masonry piers and walls have attained, on the basis of an appropriate ASTM standard test method of field-cured samples, either 75 percent of the intended minimum compressive design strength or sufficient strength to support the loads imposed during steel erection. This condition exposed employees to struck-by hazards. (2) Any repairs, replacements and modifications to the anchor bolts were conducted in accordance with 1926.755(b).

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, shall be guarded to eliminate the hazard of impalement: High Ball Erectors @ 10081Wadsworth Parkway, Westminster, CO. On or before October 11, 2012 the exposing employer did not ensure each reinforcing re-bar, which was sticking up 42 above the foundation was guarded to eliminate the hazard of impalement. Steel erector employees were working above the unguarded re-bar on the south end foundation wall. This condition exposed employees to an impalement hazard.
