The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form:      (a) Construction Site: On or about October 31, 2012, and at times prior thereto, employees were exposed to silica sand while sawing and setting stone on a chimney were the employer had not evaluated the respiratory hazard.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards:      (a) Construction Site: On or about October 31, 2012, and at times prior thereto, employees were exposed to a fall of approximately nineteen feet to the ground below due to lack of adequate training in the recognition of fall hazards and the means to control these hazards.
