29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valve: On or about October 30, 2012 and times prior thereto, employees were exposed to contaminants from improper storage of an air-purifying respirator. A half-mask cartridge type respirator used in the spray paint booth was stored unprotected and face up on a table in the paint area exposing it to airborne contaminants.

29 CFR 1910.134(h)(3)(ii)(B): Respirator inspections did not include a check of elastomeric parts for pliability and signs of deterioration: On or about October 30, 2012 and times prior thereto, employer failed to ensure that respirators were inspected for deterioration of their elastic straps. A respirator used in the paint spray booth was found to have its face straps deteriorated to the point where the ability to form a proper seal was compromised.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries: On or about October 30, 2012 and times prior thereto, employer failed to maintain ready access to a portable fire extinguisher that was mounted on the outside wall of the document storage office. The path to the extinguisher was blocked with stored document boxes and unused equipment making it in accessible for use in event of a fire.
